Eligibility
Any Logan county resident who has mental retardation or other diagnosed developmental disability may apply for case management services. In order to receive services, you must:
- Have a disability that developed before the age of 22 years
- Have a disability that is likely to continue
- If you are older than 6 years of age, you must have 3 areas of significant delay as determined through the Ohio Eligibility Determination Instrument
Support Services
Limited support may be available to families who care for their child in their home. Families may receive assistance with expenses such as respite care, home modifications, adaptive equipment, and other services for the person with a disability, which are not covered by other sources.
Supported Living
Supported living is a program that provides supports to individuals and assists them to live more independently in the community.
Major Unusual Incidents
All MUI’s must be reported to and investigated by the Logan County Board MR/DD. These incidents may consist of abuse, neglect, hospitalizations, medical emergencies misappropriation, involvement with law enforcement, relocation by natural disaster, missing persons, attempted suicide and deaths. Any incidents of abuse or neglect involving children will be reported to Children Services.
